Initial vital signs: BP = 160/90 History of present Illness:

3 hours prior to admission the patient experienced 3x vomiting of patients ingested food for 1 day associated with feeling of coldness, numbness and tingling sensation in the whole body. Past History: The patient cant remember what immunization was given to her when she was a child. The patient undergone partial thyroidectomy of left part of the thyroid gland when she was 12 years old and when she turned 23 years old the right part was removed. When she turned 51 she had a surgical removal of her gall bladder because of gallstones due to frequent intake of fatty foods. After a year, she had her menopause and was diagnosed of type II Diabetes Mellitus and started taking injectable insulin 10 units. She had her Diabetes for 12 years now and it was also associated with hypertension. Obstetric History: Patient X had her menarche when she was 14 years old and experienced dysmenorrhea. She had a regular menstruation every month. She had her first parity when she was 21 years old on her daughter. After 5 years she gave birth to her second baby. Both babies are delivered normally. Her GPTPALM is G2P2(2002). She had her menopause when she was 51 years old.
