Greenway, Suite B, Derby, Kansas 67037, legally described as Lot 3, Block A, K-15 Plaza 5th Addition, Derby, Sedgwick County, Kansas. Now therefore, know that the undersigned, for and in consideration of the sum of $0.00, receipt whereof is hereby acknowledged, does hereby waive and release any and all lien, and claim or right to lien on said above described building and premises under the statutes of the State of Kansas, relating to Mechanic's liens, on account of labor or materials, or both, February 28th, 2014.
