NOTRE DAME PREPARATORY & MARIST ACADEMY GOLD HELMET ORDER

Please complete the order form; print and turn-in with check payable to Detroit Lacrosse Co. to Melissa Weimer. Look for me at the Spring Sports Parent Meeting on Tuesday, March 11th @ 7 p.m. We are ordering from Detroit Lacrosse Company. Helmets are one size. They are gold with a chrome mask. They should arrive 10-14 days after we get the order in, which I want to complete by 3/12/14.
