Judge Leonard’s standard paragraph for continuing medical maintenance (CMM): opioids

Dr. ___________________ is authorized to provide claimant with continuing medical

maintenance in the form of office visits and prescription medications related to treatment of this
injury. This authorization is subject to the rules, limitations and requirements of the Oklahoma
Guidelines for Prescription of Opioid Medications for Acute and Chronic Pain. Claimant is
currently taking opioid medications, and further prescribing of opioids shall be made in
compliance with the Guidelines. This case shall be scheduled for review in six months to
determine whether use of opioid medication should continue pursuant to Guidelines, section IV
(G). This order may be modified or rescinded by order of the Court after hearing on the
application of either party.
