This authorization is subject to the rules, limitations and requirements of the Oklahoma Guidelines for Prescription of Opioid Medications for Acute and Chronic Pain. Claimant is currently taking opioid medications, and further prescribing of opioids shall be made in compliance with the Guidelines.
