    Requirements for the Renewal of Certificate of Accreditation of Contractors and Consultants Materials Engineers after Three (3) Years

    from the Date it was Issued, and the Reinstatement of Accreditation of Government Materials Engineers The following Documents shall be submitted to the Chairman, Accreditation Committee for Contractors and Consultants Materials Engineers, Attention: The Director, Bureau of Research and Standards, EDSA, Diliman, Quezon City. I. For Renewal: a. Letter request of the concerned Materials Engineer signifying intention to renew his certificate of accreditation. b. Original certificate of accreditation. c. Certification/affidavit as a proof that the concerned engineer is not connected with any government office. II. For Reinstatement: a. Letter request of the concerned Materials Engineer signifying intention to reinstate his accreditation. b. Original certificate of accreditation. c. Certification as a proof that the concerned engineer is no longer connected with the government signed by the head of office or administrative officer of his previous government office.
