CHECK LIST FOR REGISTRATION 2014-2015

Please follow all instructions carefully.

Download and complete the Registration Form for the 2014-2015 School Year. Download and complete the Parent Contract for the 2014-2015 School Year. Download and complete the Smart Tuition Form for the 2014-2015 School Year. Download and read the 2014-2015 Student Handbook. Download and sign the last page and attach the page to your registration verifying you read and will comply with the Student Handbook. No registration will be accepted without this page. Failure to do so will negate your registration. Complete HSA Contract and return in the enclosed envelope to the HSA . Complete all forms and attach a check for the registration/academic fee. Returning students $305 New students $330 All NEW families must attach a copy of your child/rens Birth Certificate, Baptism Certificate and Immunization Record. If your child is registering for Grade 1 and older, we need a copy of the latest Report Card/s or transcript/s.

All registrations are to be brought or mailed to Mrs. Neglia in the Main Office. Please call to arrange an appointment if you are coming to the office.
