Chief complaint : Pain on the right lower
quadrant abdoment

It has been occured since 2 days before
admitted to Emergency Unit Pirngadi General
Hospital. Fever was found 2 days ago, nausea
and vomitting was also found 2 days ago. This
patient was referred from Hanna Kasih
Hospital. History of defecation (+) Normal.
History of urine (+) Normal.
Previous Disease : Unknown
Previous Medication : Paracetamol (from
Hanna Kasih Hospital)
PRESENT STATUS

AWARENESS : Alert
BLOOD PRESSURE : 110/70 mmHg
HEART RATE : 92 x/min
TEMPERATURE : 37,6 C
RESPIRATORY RATE : 22 x/min


PHYSICAL EXAMINATION
Head :
Eye : Conjungtiva anemic (-/-), sklera icteric (-/-)
Ear / Nose / Mouth : normal
Neck : tracheal deviation (-), lymph node
enlargement (-)
Thorax :
Inspection : fusiform symmetry
Palpation : Fremitus tactile right = left
Percussion : sonor
Auscultation : vesicular


Abdominal
Inspection : Symmetry, distention (-)
Palpation : Tenderness (+), defence muscular (-) Pain
on the right lower quadrant (+)
Percussion : Tympani
Auscultation : Peristaltic (+) normal

Digital Rectal Examination : perineum was normal, anal
sphincter was tight, smooth mucosal, rectal ampule was
empty, gloves : faeces/blood/mucus : -/-/-

Superior/Inferior Extremity : Normal
Laboratory Finding
Hb/Ht/Leu/Tro = 13.6/38.1/22.100/310.000

Urinalysis
Color = yellow
Turbidity = clear
Protein = negatif
Reduction = negatif
Eritrosit = negatif
Leucosite = +, 1-3
Bilirubin = negatif
Urobilinogen = positif
pH = 6,0

IVFD RL 20 drops/min
