AFFIDAVIT OF ADMISSION OF PATERNITY
I, RICHARD S. DAMASO, after having been duly sworn to in accordance with law hereby
depose and say:
1. That I am married, of legal age, Filipino citizen, and a resident of No. 27 Upper P.
Burgos, Baguio City, Philippines;
2. That sometime in February 2008, I met Jenina Gunaden of legal age, Filipino citizen and
a resident of No. 52, New Lucban Baguio City, with whom I later had a relationship;
3. That as a result of said relationship, she conceived and gave birth to a child on December
21, 2009 and we named the child, SCARLET JEAN;
4. That I am the biological father of the child, Scarlet Jean;
5. That at the time of the birth of said child, I am not legally married to Ms. Jenina
Gunaden, her biological mother;
6. That I hereby admit under oath my paternity of the child, SCARLET JEAN;
7. That I am executing this affidavit to attest to the truth of the foregoing facts and for
whatever legal purposes it may serve.
