Applicant: Give this document to your parents/legal guardians to
let them know how to submit a Parent/Legal Guardian statement
online in support of your NSLI-Y Application.

Deadline to Submit Statement: November 19, 2013, 7:00 PM ET

Dear Parent/Legal Guardian,
Your son/daughter/ward has given us your name and email address as the one (1) parent/legal guardian that will submit
a statement as part of his/her NSLI-Y application. With this information, we have sent an email message to you that
contains a link to a secure, online statement form. Please complete and submit that form online at your earliest
convenience. The deadline for receiving your statement is November 19, 2013, 7:00 p.m. ET.
This statement will not be accepted by mail, fax, or email. It must be submitted using the official NSLI-Y online form,
only.
The email message from NSLI-Y will be sent to you by "ais_accounts@americancouncils.org. Such messages are often
misdirected to spam or junk mail boxes. If you do not receive this email message, be sure to check your spam or junk
mail box before contacting NSLI-Y.
The Parents/Legal Guardians Statement is an integral part of the NSLI-Y application and selection process. We thank
you for your time and effort in support of your sons/daughters/wards application.
