Stallings Elementary: Data Analysis Form

Grade Level Team: 5

Subject: Math Date: 10.25.2013
Assessment Tool/CFA: Teacher made assessment, ExamView Generator
SMART Goal: By the end of T1, 85% of 5
th
grade students will correctly solve division problems using single-
digit divisors with remainders in the quotient.

Plan: Using CFAs to pre-test and determine leveled groups, each teacher will instruct for 2 to 3 weeks then we
will post-assess. The groups will be fluid with students moving from teacher to teacher based on proficiency.

Why did they get it?
Students were proficient in
knowing math facts to
automaticity and understanding
the division process.
Why didnt they get it?
Not proficient in math facts
and multi-step algorithms
Why didnt they get it?
Not proficient in math facts and multi-
step algorithms

Next Steps:
Continue to reinforce math facts,
do division w/ 2 digit divisors
Next Steps:
Recess math lab, practice at
home daily

Reflections: Knowing multiplication facts and understanding multi-step algorithms are necessary to
comprehend the long division process. The students who are not proficient would benefit from SES tutoring to
reinforce the division process and to work on math facts. These students just dont understand the process YET,
but they will with continued reinforcement and practice.
