HDMF Procedures

1. Fill out PMRF form. On the Membership Catergory, Check "Non-Professional",and "above 25, 000"
income. No need to submit supporting documents since this is for the purpose of updating only. Submit
this form to your nearest Philhealth office


2. You need to pay P600.00 quarterly, or P2,400 per year, on the following schedule

Quarterly Semi-annual Annual
Pay until the last working
day of the quarter being
paid for.

Example:
Period: January to March
Deadline: March 31
Pay until the last working
day of the first quarter of the
semester being paid for.

Example:
Period: January to June
Deadline: March 31
Pay until the last working
day of the first quarter of
the year being paid for.

Example:
Period: January to
December
Deadline: March 31

3. You may pay thru;

- Over-the-COunter at Any leading Commercial Bank
Philhealth Offices
- Philippine Postal Corporation
- CIS Bayad Center
- E-Soluzione
- MLhuillier Phils, Inc.
- LBC Express
- SM Retail, Inc.


Payment Stub /form will be provided by Payment Institution


You may also make payments thru Kuya Rodel, but kindly ensure the following;
-your forms should be duly accomplished in 2 copies,
-give only the exact amount as indicated in the form, and put them in an envelope with your
name on it
-present it at 1 week days before your payment deadline date, in order for Kuya Rodel to
properly plan his schedule.
