Eligibility Requirements
1. Graduating High School Senior or Current College Student who is a native of York City, PA
Copy of HS Diploma--Required
Copy of Birth Certificate*--Required

2. Cumulative Grade Point Average (GPA) of 3.0
HS or Most Current College Transcripts*--Required

3. Proof of Family Household Income
Copy of Student Aid Report (SAR)*--Once your FAFSA has been fully processed, your individual expected
family contribution (EFC) and other relevant financial information will appear on a Student Aid Report
(SAR) that the Department of Education will issue to you and to the financial aid administrators of the
colleges listed on your FAFSA.--Required

4. Proof of Enrollment at College/University/ Vocational School
Acceptance Letter/Proof of Enrollment from Registrar at College/University/ Vocational School*
Required, Due Prior to Scholarship Payment

5. Essay from Applicant
500 words or more (typed and doubled spaced) depicting applicants plan to make the most his/her
education while assisting their community --Required

6. Headshot Photo
Reprintable Headshot of Applicant--Required
Copy of Photo ID --Examples: Drivers License, PA ID, School ID--Required

7. Application Must Be Postmarked by 7/26/14.--Required
