I, CASSANDRA BRAGADO, Filipino, of legal age, single, and
resident of 69 Jerusalem St. Multinational Village, Paraaque City,
under oath declare that:
1. I was issued an I.D. by the College of Law of San Beda
Alabang;
2. I used to keep said I.D. in my wallet;
3. Last November 03, 2013, while I was riding the LRT going to
Taft Avenue, Manila, my wallet together with my I.D. was
stolen;
4. I am executing this sworn statement, as required, to enable
me to obtain a replacement for that school I.D.
