I am a former student of the University of Santo Tomas and I took Doctor of Dental Medicine
and graduated last May 2014. I am planning to take the Dentistry Licensure Examinations on
December 2014. One of the requirements of the Professional Regulation Commission is the
complete transcript of records and certificate of completion for duty rotations.


At the same time, my review center is reminding us to take the form and attach our transcript of
records and very important documents. The deadline of filling-up forms would be on August 31,
2014.


Due to my desire to accomplish my documents and records within this month, I write this letter
to ask for my documents especially my transcript of records during my entire stay in your
university.


I appreciate your response regarding this matter.
