Details of failure

Train passed through PRND at 15.50. At16.15hrs,LP informed


that after passing N/S experienced VCB not closing with fault
F0102P1 with message VCB stuck up in off position and
stopped at Km 137/13 at 16.08hrs. LP tried to trouble shoot as
per TSD & CI/AVD advice. Later asked for RE at 16.30. RE
22902/ED left QLN at 16.48, PRND 16.57/16.58, arrived spot
17.09, attached in rear at 17.11, left spot 17.27. cleared to
STKT at 17.39. train ready after ETR at 17.55. left STKT at
18.10 after 16312. Detn-132
Rep : NIL.

Observation

Loco moved as dead to ELS/RPM. On arrival of loco at


ELS/RPM, checked and found that VCB auxiliary switch not
operated.

Investigation report

On arrival of loco at ELS/RPM on 03-02-2014, it was noticed


that the driver rod for operating the auxiliary switch of VCB
came out from rod end spherical bearing which affect the
movement of auxiliary switch of VCB and thus give the feed
back message of VCB stuck up in off position.
The photo copy of the failed driver rod and rod end spherical
bearing is enclosed herewith.

Cause of failure

Thread worn out inside the rod end spherical bearing which
result driver rod to came out from its seating.

Action Plan

1. One cyclic check is being started to check the condition


of thread of driver rod and rod end spherical bearing.
2. The firm M/s. Schneider has been advised vide letter
No. M/RS/RPM/221/II/4/51 dated 03-02-14 for further
investigation and replacement.
