Orchestra T-Shirt Order Form

All new orchestra students are required to purchase an orchestra t-shirt, if they dont already have
one from previous years. The price of the t-shirt is $10. I also encourage all parents who will be
chaperoning trips or volunteering with the orchestra to purchase a t-shirt as well. Please return this
order form to Mrs. Booth by August 29, 2014. Please make checks or money orders payable to
Wakefield HS Boosters by September 5, 2014.



T-shirts are only available in adult sizes. Please indicate the size needed below.
