I release my child to participate in the dance clinic to be held at Whiteland Community High
School. I understand and acknowledge that as with any physical activity, there is a risk or
potential for injury. In consideration, I waive any and all rights for claims of damages,
demands, or loss actions, which may occur as a result of mine or my childs participation in
this clinic, against Clark Pleasant School Corporation, coaches, volunteers, or sponsors for
myself, my heirs, executors, and administrators. I attest and verify that the participant is
