OLAS Glee Club. Therefore, I have read, understood and accepted the following the terms and
conditions:

that I will be always faithful to the schedules of rehearsals and other activities of the guild;
That of I cannot come to the schedules, I will inform the president ahead of time;
That a formal resignation shall be submitted and handed over to the president signed by the Fr.
moderator if I wish to leave the guild;
That uniforms( costumes) musical pieces(clearbooks) and instruments are properties of the
OLAS GLEE CLUB, therefore, they must be endorsed to the custodian(s) upon resignation; and,
That I will manage my TIME and SCHEDULE in order to maintain a good balance of the
academic, community, and spiritual life vis a vis my membership with the choir.
