Diagnosa 1. Minimum 2 kultur sebelum terapi antibiotik: - percutaneus - tiap akses vaskuler > 48 jam 2. Peta kuman di RS Ibnu Sina? Kultur rutin di ICU? C. Terapi Antibiotik 1. Terapi Antibiotik harus dimulai dalam jam pertama ditemukan nya sepsis setelah pengambilan sampel kultur. Grade E 2. Terapi awal emperis antibiotik satu atau lebih terhadap dugaan penyebab (bakteri atau jamur) berdasarkan peta kuman setempat. Grade D 3. Terapi antibiotik dikaji ulang dalam 48- 72 jam berdasarkan data mikrobiologi dan data klinis dengan tujuan memperkecil spektrum antibiotik ( cegah resistensi, toksisitas, menurunkan biaya) Grade E D. Source Control Semua pasien dengan sepsis harus dievaluasi fokus infeksi secepatnya dalam waktu 6 jam dan menghilangkannya (drainage abses, debridement, pengambilan alat yang terinfeksi). Pada prinsipnya dilakukan tindakan yang paling tidak menyebabkan penurunan kondisi pasien. Grade E E. Terapi Cairan 1. Cairan resusitasi terdiri dari cairan kristaloid dan koloid. Tidak ada dasar ilmiah mana yang lebih baik. Grade C 2. Fluids challenge 500- 1000 cc kristaloid atau 300-500 cc koloid dalam 30 menit dapat diulang berdasarkan respon pasien ( peningkatan tekanan darah dan produksi urine) dan bukti overload intravaskular (edema paru). Dilatasi vena dan kebocoran kapiler F. Vasopresor 1. Ketika fluid chalange gagal mengembalikan tekanan darah yang adekuat dan perfusi organ, terapi dengan obat vasopresor diperlukan. Grade E 2. Baik norepinefrin dan dopamin (lewat kateter vena sentral /CVC adalah pilihan pertama untuk terapi hipotensi pada syok sepsis. Dopamine meningkatkan mean arterial pressure dan cardiac output, utamanya dengan meningkatkan stroke volume and heart rate. Norepinephrine meningkatkan mean arterial pressure karena efek vasokonstrictif, dengan sedikit perubahan heart rate dan peningkatan stroke volume lebih kecil dibanding dopamine.
Norepinephrine vs Dopamine Norepinephrine is more potent than dopamine and may be more effective at reversing hypotension in patients with septic shock. Dopamine may be particularly useful in patients with compromised systolic function but causes more tachycardia and may be more arrhythmogenic

G. Terapi inotropik Pada pasien dengan Cardiac output (CO) rendah meski telah mendapat resusitasi cairan dapat digunakan dobutamin untuk meningkatkan CO. Bila dipakai pada pasien dengan tekanan darah rendah harus dikombinasi dengan vasopressor (Norepinefrin atau dopamin). Grade E H. steroids 1. Kortikosteroid IV (hidrokortison 200- 300 mg/hari, untuk 7 hari dibagi dalam 3-4 dosis atau syringe pump diberikan pada pasien syok sepsis setelah resusitasi cairan yang adekuat dan vasopresor untuk mempertahankan tekanan darah yang adekuat. Dosis steroid diturunkan setelah resolusi syok sepsis dan dihentikan setelah 6 hari dengan tappering off. Bila pasien sepsis tidak syok jangan diberikan kortikosteroid, kecuali pasien sedang dalam terapi steroid sebelumnya. I. Kontrol Glukosa Pertahankan glukosa darah <150 mg/dL dengan infus insulin kontiniu dan glukosa. Dengan monitor ketat tiap 30-60 menit, dan tiap 4 jam setelah kadar gula stabil. Untuk mencegah hipoglikemi saat tindakan tersebut digunakan infus kontiniu D5% atau D10% dan bila bisa ditoleransi pemberian enteral feeding. J. Produk darah Berikan PRC bila Hb < 7 g/dL dengan target Hb 7 9 g/dL pada orang. Hemoglobin yang lebih tinggi mungkin perlu untuk pasien dengan kondisi khusus ( myocardial ischemia, severe hypoxemia, acute hemorrhage, cyanotic heart disease, or lactic acidosis) Jangan berikan FFP untuk mengkoreksi kelainan faal hemostasis kecuali ada perdarahan atau rencana tindakan invasif..
Berikan Trombosit bila: <5000/mm3 tanpa perdarahan 5000 to 30,000/mm3 ila ada resiko perdarahan 50,000/mm3 (50 X 109/L) yang perlu operasi atau tindakan invasif.

In summary, within the first 6 hours for septic patients, using Riverss proposed goals, obtain hemoglobin of 10 g/dL to guarantee mixed venous saturation above 70%. Once the hypoperfusion has been obtained and in the absence of special circumstances such as acute coronary illness or acute hemorrhage, a red blood cell transfusion should be made only when the hemoglobin is under 7 g/dL to keep the hemoglobin between 7 and 9 g/dL.
