MODE OF PAYMENT OF FEES
All checks and cash payments may be paid to the Philippine Pharmacists Association,Inc., located at 815 R.
Papa St. Sampaloc Manila thru Ms. Dianne Mariz R. Sosa, PPhA Executive Assistant. Official receipt will be
issued upon payment. Or you can deposit at the bank below.
BANK TO BANK DEPOSIT
Deposit to PPhA Account No 0341-020561-002 SECURITY BANK INC. Mendiola Branch or to
PPhA Account no 0020351380 BANCO DE ORO, Recto Branch. Write your name and date of deposit in the
deposit slip. You can scan your deposit slip and send by email to philpharm.natcon@gmail.com .

Confirmation of your bank deposit will be done by email. Official receipt of payment to PPhA will be issued on-site
in the pre-registration area.

CANCELLATION OF REGISTRATION FEE
If written request for cancellation is received on or before April 11, 2014, 50% of registration fee will be
reimbursed. Request for refund of registration fee after April 11, 2014 will not be entertained.
