LOCKER APPLICATION AND AGREEMENT FORM

This contract entered this _____ day of ___________ 2014, by and between ________________________________
of ___________ and a group of bona fide students of the College of Fine Arts and Design (CFAD) of the
University of Santo Tomas (UST).
AND
The University of Santo Tomas College of Fine Arts and Design Student Council 2014-2015 presented by its
President Carlo Martin Y. Dequina and Auditor Jamielyn Paula O. Tiu.
WITNESSED
This locker lease for the Academic Year 2014 - 2015, under the following conditions, which the tenants are
bound to comply with:
1. That they shall pay the amount of EIGHT HUNDRED PESOS (PhP 800.00) as Rental Fee for the
occupancy right of the allotted locker for one school year only unless revoked due to valid
reasons.
2. That they MUST use the lock provided by the CFAD Student Council given that they shall follow the
following guidelines:
In the event that a lock proves faulty, it is the responsibility of the student to return it to the
Student Council President or Auditor immediately and a replacement will be issued.
If the lock is removed or changed, the tenant will lose rental priveladges and the items
inside the locker will be confiscated by the Student Council and will be given back only
after a valid written incident report is submitted.
3. That they shall accept the locker allotted to them by the CFAD Student Council 2014 - 2015.
4. That they shall not hold the CFAD Student Council 2014 - 2015 responsible for any loss or damage
that may occur while using the locker. The CFAD Student Council swears that the duplicate key will
only be used for emergency purposes as identified and approved by the Student Council Adviser.
5. That they shall see to it that they will not use the locker in serving their purposes contrary to the
norms and principles of the University of Santo Tomas on which the following are strictly prohibited:
Prohibited drugs, which are covered under the Dangerous Drug Law
Deadly weapons and pyrotechnics
Gambling paraphernalia
Alcoholic beverages
Porngraphic materials and other terms similar to these objects
Cigarettes
Items that may be subject to disciplinary action
6. That they shall not vandalize, destroy, or cause any damage to the lockers. Any damages should
be reported immediately to the Student Council President or Auditor for repair.
7. That they should empty their locker by the end of the Second Semester (2 days after the last day of
the Finals Week) for all lockers will be cleared out and are to be repainted.
8. That any rights to use the locker would be terminated 2 days after the Finals Week of the Second
Semester; the last day of the current Academic Year 2014 2015.

AMENDMENT TO CONTRACT
The undersigned tenants have read and fully understood the terms and conditions stated above and
promises to abide by them to the best of their knowledge and ability. Their right to use the locker will take
effect after paying the rental fee within the application period and with the submission of accomplished
forms/s and other requirement/s.

None compliance to the terms and conditions stated to this agreement would mean forfeiture of their
rights to use and open the locker without any necessary consent on their part.

Note: All unclaimed items after the provided duration of vacating the lockers will be disposed of or
used by the CFAD Student Council 2014 - 2015 for good purposes.
