AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

Municipal Mayor of Lacub, Abra herein after referred to as the first party:

And

Abra Provincial Hospital represented by ______ herein after referred to as the second party.

Witnesseth:
5. That the first party is the Municipal Mayor of Lacub, Abra while the second party is
the Provincial Health Officer of Abra;
6. That the first party agrees to refer its patients for Chest X-ray and Laboratory
Examination to the second party;
7. That the first party will be responsible to pay whatever expenses and or professional
charges to each referred patient to the second party;
8. That the second party hereby accepts the aforementioned agreement and hereby
exercise and or does it work as agreed upon.
