This study examined the relationship between smoking and lung cancer incidence in a cohort of 16,383 Japanese men. It found that current smokers had a 4.9 times higher risk of lung cancer compared to never smokers. Ex-smokers had a 2.2 times higher risk, but their risk decreased the longer they had quit smoking. The risk also increased with greater lifetime cigarette consumption. Quitting smoking was found to reduce lung cancer risk, with ex-smokers' risk reaching that of never smokers after 10-19 years of not smoking. The results supported previous findings on smoking's role in causing lung cancer.

Abstrak
Objectives: We conducted an epidemiological study of the relationship between lung cancer incidence and smoking, with special reference to the benefits of smoking cessation for reducing lung cancer incidence, to promote a local smoking control program.
Methods: The study was a retrospective cohort study. The population studied was 16,383 male examinees of lung cancer health examinations in 1995 in Tottori Prefecture, Japan. Smoking status from the questionnaire during the health examination was used as the exposure variable. Endpoint (lung cancer incidence) was obtained from the Tottori population- based cancer registry. A multivariable analysis using the Cox proportional hazard model was adapted for statistical analysis. The average follow-up period was 4.3 years.
Results: The hazard ratio of current smokers for the incidence of lung cancer was 4.9, whereas that of ex-smokers was 2.2. The dose-response relationship between lung cancer incidence and lifetime cigarette consumption (pack year) was determined. The ratio increased among younger subjects (under 65 years old). The hazard ratio of ex-smokers decreased with years just after quitting smoking, and reached the level of never smokers after 1019 years from smoking cessation.
Conclusions: We reconfirmed that the magnitude of risk estimates of smoking for lung cancer incidence was similar to those of previous studies, and smoking cessation was effective for reducing lung cancer risk.
1. Did the study address a clearly focused issue? - the outcomes considered, Ca Paru. - is it clear whether the study tried to, Ya. Dijelaskan bahwa perlu dilakukan penelitian tentang merokok dan kanker antara Jepang dan negara-negara barat sehingga dapat diketahui perbedaannya pada waktu tertentu - detect a beneficial or harmful effect? Merokok menyebabkan kanker
2. Did the author use an appropriate method to answer their question? - Is a cohort study a good way of answering the question under thecircumstances? - Did it address the study question? Ya, sesuai dengan pertanyaan penelitian
3. Was the cohort recruited an acceptable way? - Was the cohort representative of a defined population? Ya, - Was there something special about the cohort? Was everybody included who should have been included?
4
4. Was the exposure accurately measured to minimize bias? - Did they use subjective or objective measurements? - Do the measures truly reflect what you want them to (have they been validated)? - Were all the subjects classified into exposure groups using the same procedure?
5. Was the outcome accurately measured to minimize bias? - Did they use subjective or objective measurements? Tidak dijelaskan alat ukur dan teknik pengukuran - Do the measures truly reflect what you want them to (have they been validated)? - Has a reliable system been established for detecting all the cases (for measuring disease occurrence)? - Were the measurement methods similar in the different groups? Tidak disebutkan. - Were the subjects and/or the outcome assessor blinded to exposure (does this matter)?
5
6. Have the authors identified all important confounding factors? List the ones you think might be important, that the authors missed.
7. Have they taken account of the confounding factors in the design and/or analysis? - Look for restriction in design, and techniques eg modelling, stratified-, regression-, or sensitivity analysis to correct, control or adjust for confounding factors.
8. Was the follow up of subjects complete enough? Ya, tidak ada loss follow up.
9. Was the follow up of subjects long enough? - The good or bad effects should have had long enough to reveal themselves Subyek tidak diamati dalam waktu yang cukup lama (1995-1999) sehingga berisiko terjadi bias. Bisa saja terjadi pada saat pemeriksaan Ca paru ada beberapa subyek penelitian yang sudah berada pada rentang waktu - The persons that are lost to follow-up may have different outcomes than those available for assessment Tidak ada yang hilang dari pengamatan - In an open or dynamic cohort, was there anything special about the outcome of the people leaving, or the exposure of the people entering the cohort? Tidak ada
10. What are the results of this study? - What are the bottom line results? Result: The hazard ratio of smoking to lung cancer was 4,9. 6
- Have they reported the rate or the proportion between the exposed/unexposed, the ratio/the rate difference? Ya (CIR) - How strong is the association between exposure and outcome (RR,)? 4,9 (28 8,4) - What is the absolute risk reduction (ARR)? Quitting smoking, low level of cigarrete consumption.
11. Do you believe the results? Ya.
12. Can the results be applied to the local population? - The subjects covered in the study could be sufficiently different from your population to cause concern. Your local setting is likely to differ much from that of the study Can you quantify the local benefits and harms? Tidak
13. Do the results of this study fit with other available evidence?

Rata-rata periode follow up adalah 4,3 tahun.
