AFFIDAVIT OF UNDERTAKING
I, ERLIE D. NUNSOL, of legal age, married, Filipino citizen and a
resident of Camp 6, Tuba, Benguet, after having been duly sworn to in
accordance with law hereby depose and state that:
1. I am an owner of a passenger jitney and registered employer of Social
Security System (SSS) under Employer No. 01-1337657-9;
2. I reported Eduardo D. Gonzalo with SSS No. 01-0814708-5 on
September, 2010 as my driver;
3. I religiously paid my drivers contribution/premium however, on
October, 2010 to February, 2011 I erroneously used form RS-5
instead of R-5;
4. On March, 2011 I separated the drivers contribution/premium since I
was busy processing the transfer of documents and I executed Deed
of Sale in favor of IRENE NUNSOL dated March 14, 2011;
5. I am executing this affidavit to undertake and state the fact that I
already paid my drivers contribution from the month of October,
2010 up to February, 2011;
6. Further executing this affidavit to terminate my records at the SSS,
because I am not anymore the owner of the Passenger Jitney and for
all legal intents and purposes it may serve.
