1. Applicant laboratory is required to submit information on their participation and performance in PT activities or its plan to participate in PT
activities relevant to the scope applied.
2. If only PT plan has been submitted during application, the applicant laboratory shall submit this form during pre-assessment or/and
compliance assessment.
3. Accredited laboratory shall update all PT participations record since the last assessment or when required.
4. The original form to be submitted to assessment team during compliance, extension of scope, surveillance and re-assessment.
5. Copy of this form with respective PT report to be maintained by the laboratory.
