    Establishment Specified.
    Estimated date of Commencement of Each Contract work under each Contractor
    Specified
    Particulars of Contractors and Contract Labour Specified
    The Establishments Type of Work Specified.
    The License of Contractor is Attached
    The Declaration of Principal Employer Attached
    The details of the directors/partners (in case of companies/firms) mentioned.
    The details of the other person incharge responsible for the supervision and control of
    the establishment.
    The contractors details mentioned.
    The wok order agreement with contractor attached.
    Particulars of Treasury Receipt Enclosed.
    The Affidavit/declaration on stamp paper of Rs. 100 attached.
    The Challan/fee receipt attached.
    The amount paid as per fees applicable.
