Arkansas State USBC Youth Championship Tournament Entry

PLEASE READ CAREFULLY
1. The Arkansas State USBC Youth Association encourages all youth organizations to participate in the
50th Annual USBC Championship Handicap Tournament. This year’s tournament will be held in
Jonesboro, Arkansas and we look forward to traveling to this area for the tournament.
2. Please follow the following guidelines in filling out the entry forms.
‘A. Print or type the entry forms.
B. Do not use the entire space on entry for recording the bowler information, Please provide enough space
for possible substitute changesthai may beneeded above the bowler information.
C. Ifbowler does not have m 18 game average as of December21, 2014, please do not make any
remark or notation in the average box. A form will be sent to you to report the(1) current average,
(2) previous summer average with 18 games or moreor (3) 2013-2014 average if a December21, 2014
average with 18 or more games does not exist. This space will be needed later to record the bowler’s
average.
D. Each bowler must have a USBC membership card numberand must be submitted on entry AN
birthdays must be entered on the entry form. Birthdays are required for the tournament jrogram to
compute the division the bowler will be entered into. If you don’t have the birthday of a bowler, please
call their parents to obtain the birthday before you mail entries.
E. A team name and the name of the association name mst be recorded on the entry form
3. A league average sheet as of December 21, 2014 must accompany the entries. The league
average sheet must have a December 21, 2014 or earlier date printed on it. Sheets with a
December 22nd or later date will not be accepted unless it can be proved that this is the December 21st
average sheet. No entries will be scheduled until the correct average sheet is sent to the state office.
These entries will be moved to the bottom of the entries for scheduling. Travel league average sheets
must accompany entries if any of your youth bowlers participates in any of the travel leagues either in
‘Arkansas or out-of-state. Those that bowl on Sport Lane conditions during league play will have their
averages adjusted using the USBC Sport Condition Adjustment Chart. Please answer question about
lane conditioning above the League Official/Association Manager name on entry form,
4, Entries will be scheduled on a first come, first serve basis. No reservations will be granted by
letter, telephone, fax or e-mail before entries, league average sheets and money are submitted to
the state office. Please do not call to make such request. If you need a particular weekend, please get
your eniries in early. Be sure to select a second choice on dates.
Please make sure the correct amount of money is submitted with the entries, The entry fee is $15.00
per person per event. There is NO All-Events fee. A bowler bowling doubles event must also enter
the singles event. Team entry fee is $60.00 (for four bowlers), doubles is $30.00 (for two bowlers)
and singles is $15.00. If you want a team to be scheduled and you have only three members or
less for that team, the full $60.00 must be submitted. The same applies for doubles and singles. It
will be your responsibility to find a local bowler to complete the roster for team and doubles.
6. A map and motel information of the Jonesboro area will be sent with confirmations. Information is not
available at this time. The tournament schedule will be mailed no later February 26, 2015 to the youth
official who submits the entries.
7. Please advice the youth about our dress code in Tournament Rule 1 and USBC Rule 301b.°

CHECK LIST BEFORE SUBMITTING ENTRIES
1. Team name for each entry. Located above the team event section,
2, Association listed on the right side above the team event section.
3. All birthdays, ages, division code and USBC membership numbers listed
on each event section.
4, League Official printed name, address, telephone and e-mail listed on all
entries. (Hint: Fil this section out first and print number of copies you
need.)
5. Choice of Date ~ Please make sure you put a second choice.
6. Ifyou have a bowler who does not have an average as of December 21,
2014 please leave average box blank. A form will be mailed to you with
confirmation to report average.
7. Correct Entry Fee: Team Event (4 bowlers x $15.00) $60.00
Doubles Event (2 bowlers x $15.00) $30.00
Singles Event (2 bowlers x $15.00) $30.00
Entry Fee is $15.00 Per Person Per Event
8. Entire entry fees due with entries regardless if the team and doubles
events roster is complete or not. In other words, if you want 1 team
scheduled and I set of doubles and 2 singles, $60.00 is due for team event,
$30.00 for doubles event and $30.00 for singles event even if you are missing
a bowler at the time entry is submitted. It will be your responsibility to fill
all vacant positions on an entry.
9, Allow yourself plenty of time to fill out the entry forms before the
deadline. Deadline is January 26, 2015 and must be postmarked no later
than January 27,2015. Entries postmarked past the January 27, 2015 will
be returned. There are no guarantees of dates, The sooner you get the
entries submitted the better chance you have in getting the dates you need.
