Engage, assess, intervene, and evaluate with individuals, families, groups,
organizations, and communities.

Practice Behaviors
10.1 Substantively and effectively prepare for action with individuals, families,
groups, organizations, and communities.
I will review my patients charts, to include reading the history and physical, operative
notes, and progress notes.

I will be sure I am prepared with necessary forms for patient or staff to sign.

10.2 Use empathy and other interpersonal skills.
Be understanding of the patients.

Listen to the patients, make eye contact, and have congruent with words and body
language.

10.3 Develop a mutually agreed-on focus at work
Discus with my supervisor what the goals are for meeting patients needs

Prioritize when it comes to possible discharge dates to meet patients needs prior to
discharge.

10.4 Collect, organize, and interpret client data
Review charts, complete Discharge Planning Assessments, type Progress Notes.

Use patient chart and assessment, and orders to help patient meet goal of discharge.

Use Discharge Planning Assessments to determine potential needs.

10.6 Develop mutually agreed-on intervention goals and objectives
Help patient to have necessary equipment services to ensure success upon dischatge.

10.7 Select appropriate intervention strategies
If patient has certain needs that must be met from Coordination of Care in order for
patient to have a successful discharge, help patient meet those needs.

10.8 Initiate actions to achieve organizational goals
Complete FL2, discharge packets for patients.

Arrange transportation through Life Link.

Make sure patient needs are met prior to discharge.

Make sure all clients have needs met, appropriate equipment and services, regardless of
ability to pay.

10.12 Facilitate transactions and endings
As the patients discharge planner, I will make clients aware of actions I am taking in
regards to obtaining equipment and services.

10.13 Social workers critically analyze, monitor, and evaluate interventions.
After gathering information from assessment, ordering equipment, or making
appointments, I will follow-up on the status, document, and inform patients.
