2. What courses do you need to graduate with your AA degree? (Please see Lena Paxton to get a copy
of your Degree Audit, if you have problems. Lena is located in the Matrix in the Taylor Administration
Building on the CSI campus; you can also email her at lpaxton@csi.edu.) Otherwise, go to MyCSI

under

your grades option.

According to my graduation audit the classes I am enrolled in are the only ones left needed
for graduation.
3. Have you applied for CSI graduation? (https://www.csi.edu/forms/students_forms/gradapp/)
I waited until classes began (today) upon discretion of my advisor to apply for graduation. It will
be done by this weekend.
4. Have you completed CSIs Computer Literacy Assessment? (http://computerliteracy.csi.edu/)
According to my graduation audit, yes.

5. What are the entrance requirements for your chosen four-year institution? What are the
application deadlines? If you plan to apply for scholarships, what are the scholarship
application deadlines? Have you applied for financial aid?
I have talked several times with my advisor already. WGU accepts only general education
requirements. I need to check application deadlines, but as far as I know, I am completed with
what I have to do thus far. I have taken care of the financial aid aspect as far as entering the
school code into my FAFSA. I will be looking into scholarship especially because the school
offers a military spouses scholarship.
6. Does your chosen four-year institution require Praxis 1 (or other tests)? What scores are
needed? What are the dates for Praxis 1 (or other tests)? Do these tests have to be completed
before the application deadline? If so, how long does it take to process test scores?

Praxis Website: www.ets.org

From my understanding and talking to my advisor, no. However, I will not start
until February with them because it will take a month for them to square away my
transfer credits. I will need to contact them in December for more finalized
transfer plans after I graduate at CSI. This form has been a good reminder to
maybe get in touch with them again this coming month to touch base.
