JUDGMENT

The defendant, Peachtree Hospital Association, d/b/a Peachtree Nursing Care,

having been regularly served with process, and having failed to appear and answer the
plaintiffs Complaint filed herein, and the default of said defendant having been duly entered,
and it appearing by the affidavits of plaintiff that plaintiff is entitled to judgment herein.
IT IS THEREFORE ORDERED AND ADJUDGED, that the plaintiff have and recover
from the defendant, Peachtree Hospital Association, d/b/a Peachtree Nursing Care, the sum of
$12,000.00 plus interest therefore from and after October 10, 20 , until paid, together with
costs in the sum of $343.75.
