AFFIDAVIT OF ACKNOWLEDGEMENT/
ADMISSION OF PATERNITY
I, LEONARDO C. BALAOD, of legal age, Filipino, married and a resident
of Compostela Valley Province, Philippines, having been duly sworn to
pursuant to law, depose and say:
1.

That I am the biological father of Jake Quinto, who was born

outside of wedlock on May 12, 1989, at Barangay Moncado (now Villarica,

Babak, Samal City);
2. That the biological mother of Jake Quinto is ZENAIDA V. QUINTO,
as evidenced by my sons birth certificate issued by the Office of the City
Civil Registrar of Samal City under Registry No. 89-310;
3. That I hereby acknowledge accept and recognize Jake Quinto as
my son.
4. That I am executing this affidavit to attest to the truth and
veracity of the foregoing facts and to apprise the Office of the City Civil
Registrar and other concerned offices and agencies of my admission of
paternity in favor of the aforementioned child, and for whatever legal
purpose this affidavit may serve.
