I have wanted to become a teacher since I was in Kindergarten. I have been babysitting since I was 15 years old. I babysat for my
cousins for three summers during high school and before attending college. I graduated from Southwest Wisconsin Technical
College with an Associates Degree in Early Childhood Education. I am currently attending the University of Wisconsin Platteville
for a Bachelors Degree in Elementary Education. I also have a double minor in Early Childhood Education and Special Needs and
Inclusion.
During my experience at SWTC I have had the opportunity to work with Head Start, the Infant and Toddler, 2 year old, and the 3,
4, and 5 year old day care on the SWTC campus, and the Special needs classroom. During my experiences at UW-Platteville, I
have had the opportunity to tutor students in 1 st and 4th grade. I have been able to do my pre-student teaching with Kindergarten
students. During my pre-student teaching experience I was able to see how RTI is implemented in the classroom and the behavior
management of the Responsive classroom. I also volunteer in the Kindergarten classroom at the Highland Elementary one to two
days a week. I have taken classes at SWTC and UW-Platteville that focus on the development of children, providing
developmentally appropriate practices, guidance and assessment, strategies for effective inclusion, and have learned how to prepare
lesson plans.

School Location/Setting
Dodgeville Elementary School is located in Dodgeville Wisconsin. It has two elementary schools, one middle school, one high
school, and 7 Pre-schools.
Demographics
Dodgeville Elementary School has 442 students currently enrolled. 92.1% are white, 3.6% are African American, 2.7% are Asian,
and 1.6% are Hispanic. Dodgeville Elementary school has 11.8% students with disabilities, 37.8% that are economically
disadvantaged, and 0.9% limited English proficient.
Student Performance Trends
~Reading Achievement is at a 32.4/50, Math Achievement is at a 41.1/50
~Reading Growth is at a 38/50, Math Growth is at a 31.50
~Reading Achievement Gaps are at a 26.1/50, Math Achievement Gaps are at s 28/50
~Attendance Rate is at a 76.3/80
~3rd Grade Reading Achievment is at a 13/20
~WKCE 2011-2012 Reading- Dodgeville: 42.1% State: 35.8% and Math- Dodgeville: 61.6% State: 48.1%
District Goals
*The district will conduct a comprehensive study of the use of existing buildings in order to determine both long and short term
goals
*Updating the district technology plan in order to provide improvement in the utilization of technology in teaching and learning
*Provide high quality instruction and intervention to match the needs of the students. This goal includes having all students read at
or above grade level and assesses student math performance and recommends goals for students.
*To provide students with access to programs and classes to prepare them for the next level of their education. This will include a
focus on early interventions, differentiated instruction and gifted programming where needed.

Initial educators must submit Step II, A-E, to a PDP Team for Goal Approval.
Step III: Annual Review of the PDP
Annual reviews are done in years two, three, and four and will be submitted as part of your completed plan in the last year of your
licensure cycle.
Return to Step II E and enter completion dates for each activity completed.
Professional Educators must complete a five-year plan with three annual reviews.
Write a reflection in year two summarizing the progress made towards completing your goal.
Indicate, in your reflection, how you grew professionally and/or how your professional growth had an effect on student learning.
Write any revisions to your objectives or activities in the revision area.
Return to this step annually until your PDP is complete and ready to be submitted for verification.
Initial Educators may complete a 3, 4, or 5 year plan. One annual review is required for a three-year plan, two annual reviews are
required for a four-year plan, and three annual reviews are required for a five-year plan. If there is a substantial change to your GOAL,
you must complete and resubmit section II A-II E to a team for review and approval of the new goal.
