Pursuant to Section 89 of the Tax Code, we are writing to notify your good office that Conchita Chuah
Khoon Chu born on 18 October 1922 (91 years old) with BIR TIN No. 116-198-505 died on 26 September
2014. Before her death, the decedents domicile was at 12 Mustard St. Valle Verde 5, Brgy. Ugong, Pasig
City.
On behalf of the legal heirs of the decedent, (complete names of all legal heirs) and pursuant to Sections
90 and 91 of the Tax Code, we will file and pay for the estate tax liabilities of the decedent within 6
months (26 March 2015) from the time of her death.
