CHECKLIST FOR MEDICATION STEPS (A SUMMARY)

CONTENTS

1. Ensured Registered Nurse was in place


2. Washed hands
3. Prepared Equipment
Trays/containers/cups (Ensuring it was clean)
Kardex/MAR and nurses medication sheet
Water for patient
Medications/oral/IM/SC
4. Checked Medication against Kardex/MAR and nurses
medication sheet (with Registered Nurse)
Ensured the six basic rights were followed (FIRST CHECK)
5. Calculated and correctly removed medication from container
6. Did SECOND CHECK after calculating medication (with
Registered Nurse)
Ensured the six basic rights were maintained
7. Checked patients armband/ask patients name to verify correct
patient, in absence of armband, verify with nurse
8. Checked medication against Kardex (THIRD CHECK) with
Registered nurse
Ensure six basic rights were adhered to
9. Informed patient about the medication,
with emphasis on the reason for medication/The therapeutic
effects
Possible side effects (what should be done if they occur)
10. Correctly Administered Medication /ensure patient swallowed
11. Washed hands and Correctly Documents
12. Monitor patient for effectiveness or side effects within half
hour, document findings

Steps in Completing Span of Duty


Period of Span 8am-12noon
1. Handover patient to examiner at 8am
A. A brief history on patients chief complaints, reason for admission, date/time,
PSH, PMH
B. Receival notes objective and subjective data highlight important data that are
affecting patients
C. Physical Assessment General head to toe or systems; focusing on the areas of
abnormalities (abnormalities in both receival notes form the basis for Prioritizing
diagnoses)
D. Present Priority Needs to examiners (Based on abnormalities found in receival
notes and physical assessment) a minimum of three diagnoses for each patient

Continue to work with both patients, updating notes and Care plans
Inform examiner on the skills you have to perform and the time slots for them
Perform skill one, (with examiner watching) update notes, care plan
Perform skill two, (with examiner watching) update notes and care plan
End of period 11: 00-12:00 present notes and care plan to examiners (all areas

of the care plans are to be filled in)


J. 12:30pm Oral questioning on patient, condition, medications etc
K. 1pm - Results of examination
L. Closing off of notes
