AFFIDAVIT OF LOSS

I, FRANCISCO BALACANAO, also known as MIKO FRANCISCO

BALACANANO, of legal age, single, Filipino and a resident of 4768
Marsfield St., San Diego, California, USA, after having been sworn to in
accordance with law, do hereby depose and state:
1. I am the absolute and registered owner of a parcel of land
located at Rosario, Cavite covered by TCT No. T-204302 of the
Registry of Deeds for the Province of Cavite and more
particularly described as follows:
A parcel of land (Lot B-1 of the subd. Plan, Pcs-04-0002123
being a portion of Lot B, Psd-65855, LRC Rec. No. 5964),
situated in the Poblacion, Mun. of Rosario, Prov. of Cavite, Is.
Of Luzon. x x x x containing an area of ONE HUNDRED FIFTY
TWO (152) SQUARE METERS, more or less.
2. As the registered owner, I had in my custody the original
Owners Copy of TCT No, T-204302 and I kept the same in my
residence at the abovementioned address;
3. However, sometime last month, I looked for the said title from
the cabinet where I kept it but I could find the same. I
searched my entire house but despite diligent search I could
not locate the same. All search proved futile and by these
presents, I hereby declare the same as lost;
4. I hereby certify that I have not sold, transferred, assigned,
donated, encumbered or mortgaged the property covered by
TCT No. T-204302 to any person. Neither has said certificate of
title been cancelled for whatever reason by the Registry of
Deeds for the Province of Cavite;
5. I execute this affidavit to attest to the truth of the foregoing
facts, to request for the issuance of another owners copy of
TCT No. T-204302 and for whatever legal purpose it may serve.
