GlobeMed at Loyola University Chicago Howard Area Community Center Partnership Action Framework October 2014 to August 2015* This PAF describes the commitment to collaborate between GlobeMed at Loyola University Chicago and. Howard Area Community Center (HACC) from October 2014 to August 2015. Objectives for the projects listed below might be added or modified throughout the course of the year as deemed appropriate by both partners involved and as need dictates. *GlobeMed staff will not officially be volunteering at Howard during winter break 2014-2015. Individual staff members can continue volunteering if they desire based on availability and approval from Howard coordinators. *Bimonthly staff volunteering will only be from October 2014-April 2015. GlobeMied’s GROW interns will directly be iivolved with HACC during summer 2015. Project Description + Involvement with SHARP After-school Program (Fall 2014-> Spring 2015) ‘© Bimonthly volunteering (groups of 4-5 volunteers) " GlobeMed will lead nutrition classes, cooking workshops, and sports/fitness activities in the after-school program under the supervision of Kathryn Wong (SHARP coordinator) in efforts to promote healthy living and reduce the incidence of childhood obesity in the Rogers Park area + GlobeMed local partnership committee will collaborate with Kathryn and review their lesson plans prior to nutrition/cooking workshops + The volunteering will run from 4:30-5:30 PM on week days at the various after-school program locations, with the exception of cooking classes which run longer © Allgroup of volunteers will be accompanied by either the director/assistant director of local partnership or a local partnership committee member for guidance and consistency © Volunteers will have the option of reaching the after-school site early to tutor students involved in the program from 3:30- 4:30PM during homework time before after-school program activities offically begin + Field Trips & Smaller Projects ‘© GlobeMed will organize field trips throughout the year that will promote community bullding/mentorship and provide more opportunities for the kids to engage in physical and nutritional health activities = Tentative ideas: * AnIES tour at Lakeshore campus and possibly involvement with the Woodstock campus or Loyola’s Farmer's Market to expose the youth to gardening and its benefits in terms of nutrition * Bringing a group of kids from Howard over to Loyola for Christmas on Campus—fun and community building! Chicago Howard Area Community Center * Collaborating with Loyola’s Damen Student Center to arrange a health- related movie screening and discussion specifically for the youth from Howard and possibly a rock-climbing wall day to promote fun fitness activities ‘+ Using sports equipment from Loyola and arranging game days where youth from Howard can engage in physical fitness activities/sports with GlobeMed staff at the beach or on Loyola's fields © GlobeMed will aso help with gathering information and planning/setup for Howard ‘Area Community Garden in the spring semester as is necessary © GlobeMed will look into grant-funding as a possible source of raising money to help ‘expand the programs at Howard Project Measurement ‘+ GlobeMed and Howard Area Community Center will work together to develop a measure of project impact (© Ideas: consistent surveys to measure how the nutrition/cooking classes impact the participants, garden journal for the summer to track what the youth are learning through community gardening "Through these surveys, both organizations can work together to improve any limitations or areas that need to be addressed ‘Communication + GlobeMed expectations: © GlobeMed at Loyola expects to have bimonthly updates with contact person (Kathryn Wong). This could be in the form of meetings, emails, or phone calls, © One meeting (in-person) atthe beginning of the month for reflection and discussion of future plans © Having directors of programs visit GlobeMed general meetings at least once as a guest speaker to share their personal experiences working with the programs and Howard youth in general + Partner expectations: © HACC agrees to update GlobeMed volunteers via email at least once a month on the partner’s work, and progress towards project goals, © Meet with GlobeMed volunteers as needed to support collaborative projects Project (Objectives) * GlobeMed Chapter expectations: © Maintain effective communication between GlobeMed and Howard Area Community Center © Assist Howard to carry out its mission in the community * Help lower the incidence of childhood obesity in the Rogers Park area by supporting programs that promote healthy living and increase access to fresh food GlobeMed at Loyola University Chicago Howard Area Community Center ‘© Keep members consistently involved based on their avallability © Engage with more members of Howard and surrounding community ‘+ GlobeMed Partner expectations: © Overall management of the SHARP program®, including interactions between GlobeMed volunteers, other collaborators and community residents Staff and additional volunteer support for SHARP and other HACC programs © Commitment to communicate and meet with GlobeMed as needed to support the shared program goals Ongoing commitment to fund and equip SHARP program components © Overall responsibility for tracking program outputs, outcomes and impact (program. evaluation) of the SHARP program “SHARP (Seeing Healthy Active Rogers Park) includes nutrition education, physical activity, cooking classes, and community gardening for youth in HACC’s programs. GROW Internship (Summer 2015) + GlobeMed Chapter expectations: © Twoto three GlobeMed members (numbers can vary) will volunteer during the summer * 1-2Grow interns each month for May, June, and July (rotating interns) ‘Assist in tending Howard Area Community Gardens Assist in other programs (ex: Cooking and Gardening Club) as needed ‘Assess and evaluate how projects are going, help draft new PAF Interns in charge of any involvement with Loyola's Farmer's Market coco + GlobeMed Partner expectations: ‘© Provide guidelines and assistance for GROW Interns on projects. © Update GROW interns on the projects happening within SHARP.
