    ANNEXURE D
    We, (1) AHMED IQBAL HUSSAIN S/o Ahmed Hussain, aged about
    49 Years, and
    (2) ATSATH W/o Ahmed Iqbal Hussain, aged about 40 years, residing
    at 323, Malandur Road, Right Side Uppahlli, Chikmagalur Taluk & District,
    Karnataka State, solemnly declare and affirm the following :-

    1. That we submit that we are both Indian citizen by birth and domicile.
    2. That we got married on 10/05/1996 at Furkhaniya Shadi Mahal,
    Chikmagalur, Chikmagalur District.
    3. That our marriage was solemnized according to Muslim custom and
    Rites and we are living together as married couple.
    4. That our joint photograph is affixed below.
    We swear in the name of God that the acts stated above are true and correct
    to the best of our knowledge and belief.

    Verified at Mangalore on this 30th day of October of the year 2014, that
