Release, Waiver, and Assumption of Risk

To be signed by all parents or guardians of a minor child or by the participant if they have
reached the provincial age of majority.

We (I), _________________________________hereby acknowledge and agree that in

consideration of ___________________________ being permitted to participate in the
(name of participant)
activities of:

Osler Preschool or Indoor Playground or any activities pertaining to and offered by the
Osler Family Resource Centre or Osler Interagency Group (Herein called the Osler
Resource Centre).

1. We (I) do hereby release the Osler Resource Centre, it’s Members, Officers,
Directors, Employees, Volunteers, and Independent Contractors from all liability,
claim causes of action of any kind whatsoever in respect of all personal injuries,
loss of life or property losses which our daughter/son/ward (I) may suffer arising
out of the activities of the Osler Resource Centre, notwithstanding that such
injuries or losses may have been caused solely or partly by the negligence of the
Osler Resource Centre, it’s Members, Officers, Volunteers, Employees, or
Independent Contractors.

And We (I) do hereby agree and acknowledge that we (I) freely and voluntarily assume
all the risks and hazards for our daughter/son/ward (myself).

2. That we (I) have carefully read this Release, Waiver, and Assumption of Risk
agreement that we (I) fully understand same, and that we are (I am) freely and
voluntarily executing same.

3. That we (I) understand clearly that by signing this Release we (I) will be forever
prevented from suing or otherwise claiming against the Osler Resource Centre,
it’s Members, Officers, Directors, Employees, Volunteers, or Independent
Contractors.
