Withdraw and Request for Records Process:

Parent kmail/email the students teacher to request in writing the withdrawal


of the student.
1. Include reason of withdraw, new school name, address and phone
number.
2. Parent/Teacher conference call to discuss the withdrawal request.
For transcripts/records requests:
1. The new school needs to fax/mail/email Request for Records to:

**Michigan Great Lakes Virtual Academy has 30 days to release student records to the new
school. However, in most cases it will not take that long.
