    NOTICE TO ALL CIVIL SERVANTS AND DISCIPLINED SERVICES OFFICERS

    All Civil Servants and Disciplined Service officers who wish to change their out-patient facility have the opportunity to do so between 26th Nov, 2014 and 17th Dec, 2014.
    Lists of accredited healthcare facilities have been availed to the respective Heads of Human Resources in the Ministries, Government Departments, the Inspector General of Police, County
    Commissioner Offices, all NHIF Branches as well as the NHIF website.
    Please ensure that you fill in and the remit the change of facility forms before 17th Dec, 2014.
    NB: Attach copy of National ID to the change of facility form NHIF 38.
