February 6th and 13th Open House Enrollment Sessions

Coordinated Care Services, Inc. is holding an open house for enrollment in the NY
State of Health Marketplace. The open house will be held Friday, February 6th and
13th between the hours of 8:30 am 7:00 pm. Our certified and trained
Navigators will assist individuals in applying for health insurance through the NY
State of Health Marketplace. Please come to 1099 Jay Street, Building J, 2nd floor
for assistance. No appointment necessary. Bring the following items for each
person in the household:
Birth dates
Social Security Numbers
Citizenship Documents, Certificate of Naturalization, Permanent Resident
Card
Drivers License or NY State Identification Card
Most current Federal Income Tax return or income verification for the last 4
weeks
If you already began the enrollment process at home, please bring your
sign-in information (username and password) from the NY State of Health
with you
Attention: The enrollment office is Fragrance Free. Thank you for not wearing
any of the following: cologne, perfume, after shave lotion, perfume, perfumed
hand lotion, fragranced hair products and/or similar products. We appreciate
your cooperation.
