LETTER OF FINAL WARNING

The intent of this notice is to inform you that your performance and personal behavior has not
been satisfactory for the reasons indicated below and to provide you with an opportunity to
correct this situation. If this situation is not corrected, you will be subject to further disciplinary
action up to and including termination.
1. Mr. Pagares previous appraisal reports reflects his inter- personal relation with crew and
officers, for various issues with previous ship staff.
2. Mr. Pagare had been verbally warned by me in presence of Bosun for his misbehavior with
fellow crew member during discharging at Chah Bahar on 21st March 2014.
3. Mr. Pagare has to improve in his performance during vessels routine operations and has
move himself, from the manifold watch to assist fellow watch keepers during cargo
operation. The same has been observed by Duty officers and other crew member.
4. Mr. Pagare should understand that the Tea Time and Lunch Time of watch keepers is a
privilege given by Officer on Watch. Tea breaks are no where required during a Cargo or
Bridge watch as per companys SMS, Indian Seaman rules, ILO or STCW.
5. Mr. Pagare should improve his Interpersonal relation with Vessels Staff and should avoid
yelling on the Department Head for being questioned about his behavior, performance.
6. Mr. Pagare should improve his attitude for respecting seniors and fellow watch keepers.
You are expected to achieve and maintain an acceptable level of performance for the duration of
employment. Failure to do so will lead to further disciplinary action up to and including
employment termination and the same will be reflected on your Final Appraisal.
You may respond to this Final Written Warning in writing and that response will be placed in
your personnel file.

above action and have received a copy of the discipline notice.
