AFFIDAVIT OF BAPTISM
I, EUSTAQUIO V. GALIGA, of legal age, Filipino, married, and a

That on the 7th day of February, 2004, I received the sacrament

of baptism at Our Lady of Lourdes Parish, Petunia Cor.,
Chrysanthemum Sts., Area B, Camarin, Caloocan City;
That on the same day, I also received the sacrament of
confirmation at Our Lady of Lourdes Parish .
The the sacraments of baptism and confirmation were conferred
to me by Most Rev. Bsp. Antonio Tobias, DD, the sponsors being
Manolo Vergara and Rose Vergara.
As such, I am executing this affidavit to attest to the truthfulness
of the foregoing statements and to obtain a Baptismal Certificate
from Our Lady of Lourdes Parish.
