Health Screenings:
Health care costs continue to rise at nearly double
the inflation rate. Many health insurance companies
are becoming proactive in containing those costs by
offering reimbursement for wellness programs.

Reimbursement
Program
The Fitness Reimbursement Program
offers money back for members who go
to the gym on a regular basis. It's a
convenient, fun, and profitable way to
get in shape. Heres how it works:
Choose a participating fitness center
from a national network including 24
Hour Fitness, Anytime Fitness, Blink
Fitness, Curves, KokoFitClub, LA
Fitness, Life Time Fitness , Golds
Gym, Snap Fitness, the Yand
independent locations.
Present your fitness ID card each time
you visit the fitness center or Y.
Get reimbursed $20 per month (to an
annual maximum of $240) for every
month you visit the fitness center or Y
at least 12 times. The reimbursement
will be deposited directly into your
bank account or credited to your
fitness center membership dues,
depending on the fitness center you
select.
The reimbursement program is
currently available in 24 states to
covered employees and covered
spouses enrolled on a participating
plan.

Improvement:
Preventive Care
Preventive care lets your
doctor find potential health
problems BEFORE you feel
sick.Preventive carecan
include immunizations, lab
tests, physical exams and
prescriptions.
Diagnostic services help
your doctor understand
your symptomsor diagnose
your illness. They may include
the same testslisted as
preventive services, but the
tests are being used as
diagnostic services. Your
copayments, coinsurance and
