HARD FURNISHING DECLARATION FORM

promise and undertake as follows:-

1. I have read the Hard Furnishing Policy and have

understood the contents thereof.

2. I have been granted a sum of Rs. ------------------ towards Hard

Furnishing facility. All items/articles (list of items given below)
purchased against this facility are the property of Hutchison Max
Telecom Ltd/ Sterling Cellular Limited/ Usha Martin Telekom Ltd/
Fascel Ltd/ Hutchison Essar who are my employers. The Income
Tax incidence, for availing of Hard Furnishing facility, will be
borne by me.

3. I hereby undertake to buy these items/articles as per company

policy on Hard Furnishing

4. Further, I authorize the company to recover the value of these

articles/ items, in case of separation, from the dues payable to
me as per final settlement.

Payment of Hard Furnishing as a Monthly Special allowance

The Human Resource Department

per my Letter of Appointment / promotion, I am entitled to Rs
______________ as Hard Furnishing. Please arrange to pay the same as
a monthly allowance, as part of my salary from this month onwards.

The tax incidence arising from the above may be deducted at source.
