LM 4-1 Accident Reporting Procedures

In case of accident or injury at your co-op placement, you should immediately do the following:
1. Get first aid treatment right away at your placement. Seek medical attention if necessary with
a doctor, hospital, dentist, or other treating agency.
2. Report all accidents and injuries, however minor, to your placement supervisor and phone
your co-op teacher.
The following information is required:
Who observed the accident or was first on the scene
What happened
When the accident happened (approximate time)
Where it happened
How the injury occurred

Important
If the accident requires medical treatment at the placement only,
the teacher and supervisor still need to keep a record of the details.
