CHARTING OF OMT
It is important to make sure there is proper chart documentation when ordering and delivering
osteopathic manipulative treatments to a patient.
ORDER FOR OMT
It is recommended that the attending physician write the order for specific osteopathic
‘manipulative treatments in accordance with the patient s condition, The physician can write the
general order on the Doctor’s Order Sheet and provide a more detailed description in the
Progress Notes as follows:

1/8/87 Osteopathic Manipulative Treatment - to be given by hospital medical staff as
noted in PROGRESS NOTE dated 1/8/87.

Date: PROGRESS NOTE
1/8/87, O.M.T., Myofascial Release with Passive Motion of the extremities to be
performed daily.

‘The physician (intern) should record any OMT performed in the PROGRESS NOTES as well as,
record an assessment of any benefits/adverse conditions stated by the patient.
If specific techniques have been ordered and the treating physician or intem feels other
techniques may be needed, this information should be conveyed to the attending physician, who
may change the prescription as he/she sees fit.
