The mather lst custody when she went to jal. On 127712014, the mather picked up the chid fora visit. When the mother picked up the chid she noticed that he had a bruise on the left side of his cheek. Later, the child sat in someone's lap and immediate jumped up and said his but When asked why his bottom hurt, Ahziya said “mommy hurt by butt" Ahziya was refering tothe father’s paramour when he sald mommy." Ahzlya hed some abrasions on his botiom. He did not state how the paremour hut him but stated that she does hit him. The mother called! the father questioning the bruising. The father stated the brusing on his cheek was from him faling. The father always slates the child has fallan when te has injuries. No explanetion was providad forthe abrasions on his bottom. The father has indicated that Ahziya sustained these injuries from falling. There is a concem that anytime something happens to Anziya, the father will say that he has fallen. Today (12-7-14) Ahziya has a ckeular bruise and abrasion on the fet side of is cheek underneath his eye. The bruise is yellow in color and appears tobe hesling. Ms. Davis advised CP's that she was aware of the allegations, as sho had alroady spoken fo the father. She stated that ‘she would like to he have PGGM Juanita Oscoola to take the child home from the hospltal and have CPI meet withthe family In the AM. CPI made contact with the nursing staf to verify that there were no serious Injuries. Ashley trom CPT gave the child an appointment for 12-9-14 @ 2pm. Officer L. Spagenski Hollywood Police Case#33-1412-161469 CPI spoke with Child Advocate Fiona Evans 054.464.1269 and sho advised that on September 9, 2014 protective supervision ‘was erminated and the case closed with the father having custody of the minor child. The mother was granted unsupervised day visits In a public place. The mother mey algo gee her chikiren unupervised visitaion in a publi place once a waek, on @ Saturday from 10am - Spm. The mother's visit may be increased as agendum between the parties. He denied being harmed by anyono; he stated the daddy, mommy and Anallz (stepmother) are all very nice to him. He stated to CP! that he had to go to the hespital to show the dactor his boo boo. ‘CPI met with the PGGM Jusnita Osceola NEEEEEBnd sho advised CPI that she has no reason to believe that ether parent would do any intentionel harm to this child. She shared that the father and child use to five with har bofore he became ‘married and he was nothing but loving toward the child. She stated that the stopmother Is very structured when it comes to food ‘and ruies in the home, but she is not abusive. The PGGM advised that sho Is normally the one making the vistation exchange {or the child and the mother. She stated that when the mother asked for the child to his sibling's birthday party, she want to pick the chid up and notced his face and the father explained that he fell down white playing. Tha PGGM stated thatthe child Is. ‘always falling while playing and getting boo boos. (CPI met with the father at home with LEO and he shared that ne was work whan tho child fell down. He stated that his wife ‘shared with him that the child was climbing on his wagon and after being fold several times to get dawn, he fell and hurt his face. He shared with CPI that he trust his wife wih his son and they have a very loving relationship. He statod that he likes to ‘spol the child and the stepmother provides a balanced structure in the home. He stated that he shared with mother about the child's injury and she insisted on taking him to the hospital. The father also shared with the mothar that child had some difficulty with constipation and she gave him ice cream even afler knowing he is lactose intolerant. V.__ Overall Safety Assessment EXTENT OF MALTREATMENT: Anziya was found inthe lobby of a hotel unsupervised. CIRCUMSTANCES SURROUNDING MALTREATMENT: (On 01/19/2014, Ahziya was found unsupervised inthe lobby of the hole! she was staying at with Mom and her other siblings. ‘Ahziya left the room on the fih floor ofthe hotel went down the elevator tothe first floor of the hotel where she was found. Ahziya was away from the room for appraximetely thidy minutes. Mom was not actively looking for Ahziya as Mom was in the room on the fith floor passed out from drinking. Mom's 17 year old friend was looking for Ahziya. Its unknown how often Mom ‘drinks or If she fs otinking to the point of intoxication. In the room there was clothing all over the place as well as a Ciroc Vodka Botte that was 75% full on the table. The Ciroc bottle was within reach of the other children Inthe room and could have been ‘opened by one of the children, Mom was arrested following this incident. ‘CPI spoke with Oficer V. Moleon idiK3364 Hollywood Police. He shared with CPI that the mother was arresied and the fathor ‘came on the scene to take the children home. The mother was with nother 17yr old person from the tribe. He stated that he ‘mother was passed out end the vodka was in reach ofthe children, (CPI met with Kristine Deering fom Saminole Family Services and she advised that she is very familiar with the family. She ‘2dvised CPi thatthe father Alberto isa bit more stable than the mother and he is normally the primary caregiver tothe children. ‘She composed a safety plen for the family to agree to have the father Alberto keep his twins wth him and the matemal ‘grandparents and to have the mother’s other two children go to Big Cypress with the matemal grandmother, unt further notice, CPI mat with the PGM Rosemarie Rescigno 954-824-2778/CPI mol the PGF Alberto Delgado and they beth feel that that thelr ‘son is doing a great job with the children and they are wiling to halp care for and keep the children safe.
