28 Years Old Male
American Indian
Maxton, NC
9th Grade
Probation- Breaking and Entering (Has to pay probation

fines and pass 3 drug screens)


Medicaid
Hangs Sheet Rock
Lives with Parents
Single and Never Married
Five Children- 4 Girls 1 Boy

Back Ground
Foster Care at age 9- Ran Away at 15 to Live with parents
Ran back home to defend mother from father
Parents had Bad Nerves due to death of younger brother
Child Died from injuries suffered from a household window infront
of parents. He saw his brother stuck in the window bleeding from
the cut.

Problem Statement
Cannabis Use/Dependency
Not an issue- 8 yrs old
Generalized Anxiety Disorder
History of Cocaine Dependency
16 years old heavily for 4 months straight
History of Conduct Disorder
Hyper, Severe Temper Issues, Oppositional, Restless
Incarcerated for several breaking and entering charges, involuntary

manslaughter, other crimes due to cocaine addiction

Learning Disorder
Receives SSI and not able to work full time because of Disability
History of Injuries Due to Accidents

Level I Outpatient Services
Group Therapy- 20 hours
Individual Therapy

Strengths and Limitations


Everything, I can do anything, fix anything, build, frame,

hang, and lay concrete.


Communicates with people well and works well with
hands
Limitations
Coping Skills, Problem solving skills, learning to monitor

symptoms, Relapse Prevention Skills


Works late in afternoon so the classes might be difficult to attend

Assessment Goals
Outpatient Therapy to assist with coping skills,

identification of triggers, problem-solving skills,


identification of symptoms, learning to monitor symptoms,
and relapse prevention skills.
Also could benefit from learning relaxation techniques,
having the ability to identify his thoughts and feelings to
assist with his behaviors as well as his decisions
-Personal Goal: Pass a drug test

Biases
Client reported he had been smoking since age 8 and it

was not a problem he could quit whenever.


Single with 5 children

Clients main goal was to pass drug test and get through

treatment for probation
