CASE

PRESENTATION

Admission
Client was admitted to the facility after
being in the hospital
Client had trouble ambulating and was
sent to the facility while she completed
therapy

Client

Interview
To conduct this interview I referred to the
conceptual framework of Person in the
Environment
I took into consideration her home
environment and her new environment
within the facility

Resident admitted to past drug use


during the initial assessment
She voiced financial concerns about
maintaining her bills at home and paying
for the treatment in the facility. (stated
several times I have a car payment.)

Resident seemed eager to return home


where she resides with her mother
Resident states concerns about taking
care of her mother (she is not fully able to
bend and is not supposed to lift much
weight)

Problem Statement
The resident is in the facility due to limited mobility. She has
two other sisters, but she is responsible for providing for her
mother. Her sisters will not help her and she is not sure she
will be able to do it on her own. Resident thinks about her
predicament and often seems depressed. She reminisces on
past events, such as her history of drug use in the 70s and
the abuse she went through with her husband.

Assessment Goals
The goal of this assessment was to see what could be
done to make her most comfortable while she stayed
in the facility and to see what goals she had for
herself.

Motivational Interviewing
Motivational interviewing was used to help her stay
positive about her situations. She discussed praying
and that was encouraged because she said it helped
her feel better.

Biases and Differing Values


Initially she seemed really upbeat and that lead me to believe
that everything was fine with her.
Our values are different because we have different
responsibilities and because of our age and physical abilities

Life Experiences
The client seemed sad because she was used
to being independent. As she grows older she
can not do the things she was used to doing
for herself and her mother. It causes her to
have to depend on other people for help.

Recommendation
The staff recommends that she continues to be motivated to
remain upbeat and make the most of her therapy. The financial
manager spoke with her to ease some of her worries about her
financial burdens. It was also recommended that her sisters be
spoken to about how she feels and also getting home health
to assist her and her mother once she returned home.
